education activities, if you work(ed) at a college or university), and other scientific activities in university,
scientific societies, social communities, etc.
(5) A list of two references (including the names, phone numbers and e-mail addresses)
(6) Self-assessment of applicant’s ability to read, write, and speak Japanese
(7) List and copy of all publications (PDF files to be submitted on a CD-R, DVD-R, or USB flash drive)
